Crider Health Center serves the Missouri counties
of St. Charles, Lincoln, Franklin, and Warren. If you are resident
of these counties you may be eligible for services. To access services
please follow the following directions

- The receptionist will gather information from you and refer
you either schedule an appointment for primary health care, dental
or refer you to the intake coordinator for mental health services..
- For mental health services, the intake coordinator will do an
initial phone screen to assess which services you may be eligible
for and how we can best help you.
- At that time the intake coordinator will either refer you
to the appropriate community resource or send your information
to the appropriate department in Crider Health Center
- At that point if services through Crider Health Center are
deemed of value to you, you will be asked to come in for an
intake evaluation and assessment
