What is Independent Supported Living through Crider Health Center?

Independent Supported Living (ISL) is a residential program provided by Crider Health Center to individuals with both a developmental disability and a mental illness.  The program is custom-designed for each individual to assist the individual in creating and living in his or her own home.  Each  client’s needs are met through individually-tailored support services.  This enables clients to live, work, learn and actively participate in the community.  Clients may live in houses, apartments or condominiums.  Staff may be present in the home anywhere from a few hours a day, to 24 hours a day seven days a week, depending on the needs of the individual.

  • Residents choose décor of home and individual rooms
  • Leisure activities with staff
  • Learn life skills including: cooking, cleaning, shopping, etc.
What sets Crider apart from other ISL programs?

As a Federally Qualified Health Center, Crider is able to provide peripheral services that make it uniquely beneficial.  ISL clients benefit from the array of services that Crider provides:

  • Psychiatry
  • Counseling Services
  • Primary Health Care Offices for Physicals and Sick Visits
  • Dental Care
  • Access to Crider Health Center’s own Pharmacy
To participate in the ISL program, individuals must be:
  • under case management of a regional department of mental health.
  • diagnosed with both a developmental disability and a mental illness.
  • Medicaid Waiver approved.
  • 17 and nine months of age or older
