Program Goal
The goal of the Pathways RCF II program is to provide an open, self-sufficient community life environment which will address the varied needs of the program's adult residents. These residents enjoy companionship and warm, friendly living while receiving individual care and attention for their personal needs.

Services Provided
Specific rehabilitation services have been developed that are tailored to meet individual needs of Pathways residents. These services may include:

  • Treatment aimed at teaching social skills and responsible life choices
  • Recreational activities within the home
  • Individualized case-management through Community Support Services
  • Referrals to additional community agencies
  • Medication monitoring and review
  • Crisis stabilization services
  • Leisure time activities
  • Transportation to medical care
  • Work and recreational activities outside of the home

Community Activities
Members of the Pathways programs are encouraged to seek off-site educational and work opportunities based on personal need and availability. Many of the possibilities include receiving vocational rehabilitation, GED classes, business school, or college-level learning options.

Pathways members are also referred to the Headway Rehabilitation Clubhouse in St. Charles. This clubhouse provides socialization activities along with skill-building activities designed to help integrate the members into a community setting.
